Introduction

Country Woods Adult Care, LLC., operates a community based assisted living facility known as Country Woods Adult Care and is located at: 7021 Hartel Rd, Potterville, MI 48876, in the Township of Benton.

Our Mission is to uplift and enhance your quality of life and independence, while maintaining a balanced level of personal and medical care. A qualified staff is on-site 24 hours a day, with 24/7 “on-call services” available to our residents that is provided through Visiting Physicians Association (VPA). This includes physicians, physicians’ assistants, nurse practitioners, nurses, PT, OT, mobile X-ray, etc., which allows for more at-home individualized care as needed for our residents. Further, VPA includes a professional team that will assist with a dignified transition into Hospice care.

In addition to these services our trained staff also work to provide a close family and home environment including great home cooked meals for breakfast, lunch and dinner with a well-balanced menu that is subject to change weekly. We provide a fun, yet nutritious snack time and festive holiday meals and decoration paired with activities.

Our Facility

Country Woods Adult Care has recently renovated and updated the facility and continue to exceed today’s standards in this industry, beginning with a state-of-the-art security.

We have improved the living space to include two new shared rooms, and two additional private bedrooms, for a total of 12 licensed beds. Other upgrades include:

Upgraded – zoned heating and air conditioning.
New kitchen and laundry facilities.
Ayers dry basement and sump pump systems.
Brand new septic and well.
State of the art fire monitoring and sprinkling systems.

Brand new beds and bedding.
Safe ambulatory flooring.
All new furniture in common areas.
Upgraded Admin office and Salon.
